SESSION DESCRIPTION:
This seminar will break down the documentation process into its various components ~ from assessment to care planning to writing measurable, specific and individual goals and approaches to completing the MDS. This will be an interactive day incorporating lecture, role-play and hands on aspects of care planning.



SESSION GOALS:

In the Beginning ~ Assessment
• list 5 reasons why we keep records and document patient status.
• define the role of assessment as it pertains to goals and objectives.
• list 5 sources/ways to obtain information about the resident.
• list 9 considerations in doing the client interview.
• list 7 items that may be revealed about the resident during the interview.
• will explain why we do not utilize one visit to make our final decisions regarding our resident assessment.
• will list information that would be gathered for the assessment form.
• identify the 2 types of assessment tools/forms used in Long Term Care facilities include.

Writing Goals, Plans and Progress Notes Too...
• list the 8 considerations in goal writing.
• list the 4 considerations in writing a plan.
• explain how to determine if your goal and plan will pass the state inspection.
• will list and define the 4 components of a goal and plan.
• explain the intent of progress note and list its 3 components.

Unraveling the MDS
• differentiate between the full MDS and the quarterly MDS.
• will list and describe the 5 areas of Section N - Activity Pursuit Patterns on the full MDS and the 2 areas on Section N on the quarterly form.
• describe the process for completing the Resident Assessment Protocol (RAP) module.
• describe significant change in condition as it relates to completion of the MDS.
• will complete an MDS along with the supportive documentation.

Putting it All Together
• complete a resident assessment form.
• complete Section N for the full MDS.
• write a goal and plan for the resident care plan.
• write a progress note.
• complete Section N for the quarterly MDS.
• revise the goal and plan for the resident care plan.

Merritt Parkway North - take Exit 64, left at end of ramp. Go under overpass and take first right. Masonic is on the left about ¼ mile.

91 North - take Exit 13, (left exit), go left at end of ramp, then immediate first right at light onto Toelles Road. Take right at end of Toelles Road onto Hartford Turnpike. After light, take left and Masonic is on the left.

From Hartford:
91 - take Merritt Parkway South (Route 15) to Exit 65, take a left at end of ramp. Masonic will be on right ¼ mile. Go through the grounds to the main entrance and front parking lot.

From Waterbury:
Route 84 - to Route 691 and take Exit for Merritt Parkway and 91 South. Follow directions from Merritt Parkway South above.
